The forms below are for those who plan to attend the Self Awareness Weekend. First, complete the Registration Form and send with your deposit or full payment via email or mail to the address shown below to hold your place. If you would like to Register Online Go Here. Second, complete ALL questionnaires and send to Andrea by the Tuesday before your weekend (via email or mail). If you have already registered for a SAW date and paid your deposit, you may pay the remaining balance online here. Third, read the Reminder List - it will give you all the logistic information about the weekend(i.e. times, what to bring, what to wear, etc.). This form is VERY important. Mail, fax, or email the completed forms to: Self Awareness Institute
